Discharge Coordinator - Multiple Openings

Job Description:

Rapidly expanding healthcare provider has immediate openings for discharge coordinators. Under the direction of the Clinical Supervisor, provides care coordination and discharge planning for patients needing ongoing medical surgical services. Facilitates transfers between network facilities. Places outreach calls to assure compliance with discharge plan of care. Identifies potential quality issues and assists with related research. Utilizes clinical and analytical skills to identify and coordinate follow up care as appropriate. Reviews discharge summaries and consultation reports to confirm appropriate reporting of patient safety events, critical findings including newly diagnosed cancer, and other contract requirements. Provides support to non-clinical staff on clinical and coding questions.

Required:

o Licensed Practical Nurse or Licensed Vocational Nurse, with current unrestricted license in appropriate state

o 2+ years experience in Medical / Surgical clinical nursing

o Proficient working with on-line systems

o Exceptional oral communication skills
