• Perform comprehensive patient assessments, collaborating with our multi-disciplinary team to achieve patient goals and outcomes.
• Lead with heart, nurturing care coordination and supervising a caseload of LPNs and Home Health Aides.
• Provide diligent documentation of patient journeys, capturing OASIS assessments, SOC/Admissions, recertification, and physician orders.
• Collaborate effectively with PRN staff to ensure seamless support for patients.Benefits
